It’s the gap between the specialist and your PCP doctor. We call it "Care Fragmentation," but you probably know it as: "The right hand doesn't know what the left hand is doing.”

Research into the American healthcare system highlights a few startling statistics regarding these gaps:

• Communication Failures: Studies have shown that roughly 25% to 50% of PCPs are unaware of what happens during their patients' specialist visits.
• Referral Loop Gaps: In many systems, the "referral loop" (the specialist sending a report back to the PCP) is only completed about half the time.
