Dr. Michael Luft discusses sports and back to school physicals and the advantages of annual wellnes checks. Through August 31, make your appointment at the CCMH Medical Clinic for back to school and sports physicals. Appointments are available Monday through Friday, during regular clinic hours -- appointments are required. Physicals may be scheduled as an annual wellness visit and CCMH will bill insurance. Parents should check their insurance plans. If there is no health insurance, the physical is available at a special rate of just $25 – payment must be made at the time of service. Physical forms must be completed and signed by the parent or guardian before the physical exam. Forms can be obtained from the school or at the clinic.
