Ayoub Ashraf discusses how the VA operationalized Zero Harm across the largest integrated health system in the United States — more than 1,200 facilities serving over 9 million veterans.
Rather than treating safety as a scorecard, the VA designed it as a systemwide operating discipline, anchored in focused national priorities, standardized tools and workflows, frontline engagement and psychological safety, and alignment with High Reliability Organization (HRO) principles.
